Integrated healthcare improves the patient journey…

Using sfn to help see patient flow and measure outcomes
Nurse and patient

Traditionally, patient care is monitored on a departmental basis. Integrated healthcare focuses on patient care across the whole pathway, and spans organisations that include ambulance, acute care services, residential care and primary care.

By operating collaboratively with all healthcare providers, we can deliver the right care in the right place at the right time.

The sfn Viewer lets you create a graphical representation of the patient journey, that incorporates data from different healthcare providers. This gives an at-a-glance view of the whole pathway, and makes it easy to identify the variations that reduce efficiencies.

Measuring patient outcomes across the whole pathway links all of the services in each patient’s journey. This replaces silo working and individual service metrics, and contributes to a more seamless patient journey. In turn, this gives better outcomes for the patient and affords benefits to ambulance, community services, mental health providers, hospitals and general practitioners.
