The sfn Viewer lets you create a graphical representation of the patient journey, that incorporates data from different healthcare providers. This gives an at-a-glance view of the whole pathway, and makes it easy to identify the variations that reduce efficiencies.

Measuring patient outcomes across the whole pathway links all of the services in each patient’s journey. This replaces silo working and individual service metrics, and contributes to a more seamless patient journey. In turn, this gives better outcomes for the patient and affords benefits to ambulance, community services, mental health providers, hospitals and general practitioners.
